New Clients

Thank you for choosing Tech Ridge Pet Hospital to care for your pet. To assist us in adding you and your pet to our system prior to your appointment, please:

• Download and complete our New Client Form. 
• Contact your previous vet and request your pet's medical record be sent to us. Specifically, request your previous veterinarian send the "Patient Chart with doctors notes." If your pet has had blood work or x-rays with a previous veterinarian please request these records be included as well.
